State School Entrance Health Form
For all new students, a complete state school entrance medical form must be completed by your pediatrician and returned before the first day your child attends school. For all returning students, we must receive documentation of updated immunization semi-annually through age 2 and then again at age 4. These must be completed by your pediatrician and returned after each check-up for your child. We require all age-appropriate immunizations and we only accept medical exemptions.
